Note: A signed membership application and waiver of liability must be submitted with your membership fee each year.

How do you fill in the form?

1. Download the PDF form by clicking on the link.
2. Save the blank form to your desktop. Rename the file when you save it -- for example, save the file as yourname.pdf.
3. Open the blank form in Acrobat Reader (Totally free download from Adobe.com). You may run into problems if you have an old version of Reader or if you use a clone vs. the real Adobe Reader.
4. Fill out the form by clicking in a data field and just typing as you normally would. Save the completed form to your desk top.

Note: If you're renewing a membership, you need only fill out the name fields and the fields in which the information has changed (if you have a new phone number, for example).

Even if no information has changed, you MUST sign and submit the waiver for 2011.

5. Attach the completed form to an email and send it to jodavies.sbr@gmail.com.

How do you sign the form and the waiver?

1. You can sign the form electronically. Just follow the onscreen prompts to create a signature and password for yourself. Attach the signed form to an email and send it to jodavies.sbr@gmail.com. Mail or give your check to Barbara Resendez.
2. If you prefer, you can print the form, sign it in the traditional way, and then mail it (along with your check) to Barbara Resendez.

Whether you choose method 1 or 2 to sign the form, it is important to fill in the form electronically if you can and send the completed form to jodavies.sbr@gmail.com. We're trying to avoid having to retype all your information into the roster and avoid the proofing and checking that goes along with that.
